If you are looking to establish a relationship with potential patients then there is no better way to do this. The forms give you the opportunity to collect information about future clients 24/7.

The different types of interactive forms that are available are: enquiry form, newsletter signup, feedback forms, request a call-back form.
  • The enquiry form gives clients the opportunity to contact you and ask you questions without having to call or send an email. When the form is completed it will be sent directly to your email address. There are certain fields that are compulsory (i.e their email address and comments) and if this information is not entered correctly, they will receive a message saying that the details are incorrect and need to be corrected.

  • The feedback form has two benefits: it allows clients the chance to provide you with information concerning the quality of service you have provided to them and it allows you to take the information and display it on your site as testimonials.

  • The newsletter signup forms is the perfect way to get in touch with customers whenever you have specials or promotions running.

  • The call back form gives potential clients the chance to speak directly to a physiotherapist.
After every form has been completed, a thankyou page will be delivered to the patient.
